TL;DR

Reconcile referral receipt with the clinic's actual evaluation pathway. Distinguish repeat transmissions, missing administrative information, qualified review, contact, an offered evaluation, a confirmed appointment and attendance. Give unresolved records an owner and next action. Do not report documents received as patients acquired or treat every unscheduled referral as an intake failure.

Start at the receiving queue

A recorded receipt of a fax, portal message or other approved referral transmission establishes arrival only in that receiving system. A sender's transmission record alone should not be treated as confirmed clinic receipt. Even confirmed receipt does not establish that the intended clinic team reviewed it, could contact the person or had the information needed to offer an evaluation. Begin with the clinic's authorized receiving system rather than an agency's call total.

Choose a receipt cohort and an observation cutoff. Ask staff responsible for referral intake to determine the relationship between transmissions and actual referral requests. A resent document, a correction and a new referral can otherwise look like the same kind of activity in a raw inbox count.

Keep identifiable reconciliation within approved practice systems. Marketing should receive only findings approved for its recipient and purpose. Do not send referral letters, diagnoses or patient narratives to a vendor to explain an aggregate scheduling gap.

Build the referral-to-evaluation worksheet

The worksheet is an original administrative review tool. It does not determine clinical suitability, authorization or compliance. Qualified practice staff must supply those decisions using the clinic's actual payer, clinical and jurisdictional requirements.

State to reconcile Evidence in the approved system Owner and next question
Transmission received Receipt record and intended destination Referral intake: did this reach the right clinic queue?
Request reconciled Approved linkage of repeated, corrected or distinct transmissions Intake records owner: is this a duplicate document or a distinct request?
Administrative information unresolved Missing item category without copying clinical detail into marketing reports Intake or billing: who can obtain or clarify the required item?
Qualified review pending or completed Actual status supplied by authorized staff Clinical lead: what administrative next step is approved?
Contact attempted or completed Recorded outcome and approved contact process Intake: was the person reached or is status still unknown?
Evaluation option offered Service, clinician, location and usable appointment pathway Scheduling: was there an option that matched the administrative request?
Evaluation confirmed Confirmed evaluation appointment record Scheduling: what is the current state after cancellations or reschedules?
Evaluation attended or otherwise resolved Verified attendance or documented disposition Operations: what outcome is actually supported at the cutoff?

For an unresolved record, add responsible role ___; outstanding category ___; next approved action ___; internal review point ___. Keep “declined,” “unreachable,” “awaiting information,” “no suitable option” and “unknown” distinguishable where the records support them. They imply different actions and should not all be renamed lost leads.

Separate a referral document from other prerequisites

For U.S. Medicare Part B outpatient PT, Medicare says a doctor or other eligible healthcare provider must certify the need for therapy. A marketing report should not assume that receiving a referral document resolves every applicable clinical or payment requirement. Medicare's PT eligibility information.

Have the clinic's qualified clinical and billing owners identify the actual requirements for the service and payer involved. Do not generalize that Medicare rule to every patient or convert it into a claim that all PT requires a physician referral. Public access language needs to match the actual jurisdiction and arrangement.

Add a prerequisite record beside the referral state: actual requirement ___; service and payer scope ___; qualified owner ___; evidence needed ___; point in the process when it applies ___. Keep referral transmission, certification of need, any applicable payer authorization and appointment confirmation in separate fields. A received document should not automatically mark those fields complete.

Have the responsible owners distinguish a scheduling dependency from a treatment or payment requirement. Do not turn every unresolved billing item into a pre-evaluation stop, or infer certification timing from the brief Medicare coverage summary. Record the clinic-approved next action for the actual situation.

This distinction matters operationally. A referral may be awaiting an administrative clarification even when a person is interested and a calendar has open space. Record the specific unresolved category and the actor who can resolve it. Buying additional referrals does not answer that missing-information question.

Reconcile movement rather than subtract unrelated totals

Do not subtract this month's attended evaluations from this month's incoming transmissions and call the difference lost referrals. Some evaluations can arise from earlier receipts, some transmissions can be duplicates and some unresolved referrals can legitimately remain open at the cutoff.

Track the defined receipt cohort through its actual states within the authorized system. Preserve status history where available, including reschedules. Reconcile the current disposition against that cohort. If history is incomplete, report the unresolved connection instead of assigning a presumed reason for loss.

Then group findings for management: receiving failure, missing administrative information, pending qualified review, contact gap, evaluation availability, recorded decline or unresolved history. Only use categories the evidence supports. Staff should not be scored as negligent when they could not offer the required service or the person chose not to proceed.

Fix the handoff the worksheet identifies

A receiving failure needs the relevant system owner. A recurring missing-information category may need clearer approved instructions for referring offices. A shortage of the correct evaluation type belongs with scheduling and operations. A broken public contact form belongs with the web implementer and receiving team.

W3C recommends clear success and error feedback for forms. That helps someone understand a submission, while the clinic still needs to verify operational receipt separately. W3C form notification guidance. Follow the booking incident method for a coordinated synthetic test that avoids unwanted appointments and outreach.

Use the measurement dictionary to keep document receipt, inquiry and confirmed appointment distinct. This reconciliation goes further by identifying who can resolve a PT referral's specific dependency, when that requirement applies and which evaluation step can actually proceed.

Frequently asked questions

Should the referral source be blamed when information is missing?

Record the actual missing category and check whether the clinic's instructions and receiving route were clear. Resolve the handoff through the approved process. A worksheet should identify the next action, not infer motive or fault.

Can we market directly to people in the unresolved referral queue?

Do not turn the queue into an agency campaign list. Have the responsible practice owners assess the actual purpose, communication and permitted information use. Continue administrative reconciliation within the approved process.

When can we call a referral converted?

Use an explicit event label. Contact completed, evaluation confirmed and evaluation attended describe different results. Choose the measure appropriate to the business question and retain the other states so the path remains understandable.
